The undersigned (on my own
behalf and of my heirs, personal representatives, successors and
assigns) for and in consideration of the opportunity to participate in
the 13th Annual COPS Ride to benefit the Concerns of Police Survivors,
and for other valuable consideration the receipt and adequacy of which
are hereby acknowledged releases and discharges the respective officers,
directors, employees, agents, donee, beneficiaries, benefactors, from
any and all claims and demands rights and causes of action of any kind
whatsoever which I now have or later may have in any way resulting from,
arising out of, or in connection with my participation in said event.
This release extends to any
and all claims I have or may have or as the parent or guardian of minor
child(ren), against the released parties whether such claims result from
negligence (except willful neglect) on the part of any or all of the
released parties with respect to the event or with respect to the
conditions, qualifications, instructions, rules or procedures under
which the event is conducted or from any other cause.
I am experienced in and
familiar with the operation of motorcycles and or motor vehicles and
fully understand the risks and dangers inherent in this event. I am
voluntarily participating, or allowing the minor child(ren) for whom I
have legal responsibility, voluntarily participate in the event, and I
expressly agree to assume the entire risk of any accidents or personal
injury, including death, which I or the said minor child(ren) might
suffer as a result of my/own participation in the event.

any statements or representations of anyone released thereby.
